Step 1: Find Hospitals Listed in Your Insurance Policy

Before you get hospitalized, you need to inform your insurance company about the same

- Planned Hospitalization: Intimate your insurance company at least 24 hours prior to admission by calling on <a href="tel:1800-209-5858">1800-209-5858 </a> or via email bagichelp@bajajallianz.co.in

- Emergency Hospitalization: In case of emergency, intimate your insurance company at <a href="tel:1800-209-5858">1800-209-5858</a> within 24 hours of admission

Step 2: Inform Your Insurance Company

<ul>
<li>Fill out the Pre-authorization form given by the hospital </li>
<li>Show your insurance policy and provide valid photo id proof. </li>
<li>Hospital will send the cashless request to us (insurance company) and coordinate for authorization.</li>
</ul>

Step 3: Pre-authorization

After discharge, the hospital will share the required documents with us/TPA  for approval.

- In Cashless hospitalization:
If your ailment/illness is covered in your policy, we (the insurance company) will examine the documents and release the bill amount. 

- In Non-Cashless Hospitalization:
If the network hospital is a non-cashless hospital, you can apply for a reimbursement claim. 

You need to send all the documentation required to us (insurance company) 
We will examine the shared documents and process the claim in 14 working days.

-- Important documents required
The claim form should be filled with details like the employee code/employee. Details like:
<ul>
<li>Hospital bill with break-up for medicines and others</li>
<li>Hospital bill receipt</li>
<li>All investigation/lab reports </li>
<li>Discharge summary</li>
<li>Advice from treating doctor for the medicines purchased/tests done outside the hospital during the hospitalization period</li>
<li>Doctor’s prescriptions with the duration/start date period of problem/illness.</li>
<li>Pre-& post-hospitalization bills supported with prescription/advice of treating doctor.</li>
</ul>

Step 4: Claim settlement 

We have tie-ups with a large network of hospitals, and these are known as network hospitals. You can avail cashless treatment at any one hospital if your ailment is covered in the health insurance plan. 

Cashless Process

You need to thoroughly check all that you have been billed for at the hospital before you fill the reimbursement claim form

Step 1: Verify all hospital bill details

Here is the complete list of documents you will need to attach with the claim form:
<ul>
<li>	Insurance Card/Bajaj Finserv Health EMI Card </li>
<li>	Copy of your insurance policy</li>
<li>	Medical certificate with doctor’s signature</li>
<li>	All reports including X-rays</li>
<li>	Hospital discharge note</li>
<li>  All original bills and receipts</li>
<li>	All original chemist/pharmacy bills</li>
<li>	NEFT details to show bill settlement</li>
<li>	CKYC form is claim amount is more than 1 Lac</li>
<li>	Investigation report, if any</li>
</ul>
In case of an accidental claim,
<ul>
<li>	FIR/MLC copy </li>
</ul>
Finally, a duly filled and signed Reimbursement claim form.

Step 2: Gather all Required Documents

If everything is in order, we will process the reimbursement claim in 14 days.
If we need more information, we will duly inform you.

Step 3: Submit the claim form with documents

If the claim is admissible, we will send the cheque for the payable amount to you.

Step 4: Reimbursement Claim Settlement

In case, your illness/ailment/treatment is not covered in the cashless facility, or the hospital is not a part our (insurance company) list of hospitals, you will pay the hospital bill and then apply for a reimbursement claim.
